A To-Do List for You:
1. Medical History
The most important item! We have to know all about your current health status, any medications you may be taking, any allergies, etc. You can download a copy of our medical history form by clicking on the link below. Please fill it out at your first convenience and return it to your dentist’s office at least 2 weeks before your appointment. That gives us time to follow up on any areas that need close attention.

2. Consent for Sedation
Please download and print this form and read it carefully. Each paragraph has a small line next to it on the left side. If you agree to the terms of that paragraph, please initial it there. If you have any questions or concerns, email us here or call us at (810) 278-2310. At the bottom of the form is a line for your signature and today’s date. You’ll need to sign there and have another adult sign as a witness.
