You have a patient who wants to be sedated.
Let’s plan your first case.
There are a few “administrative” items to complete before we schedule the appointment:
1. You’ll need to forward a medical history to Great Lakes Sedations. (There’s a link to PDF medical history files below). Please have your patient fill out both pages and fax or mail it. After it’s reviewed, we will schedule a telephone consultation about the patient and the procedure and we’ll discuss the sedation options available and answer any questions. We’ll also set up some tentative dates and times for the sedation appointment.
2. Next you’ll need to have a pre-sedation consultation with your patient. At this appointment you need to obtain a signed consent (available on the link below), answer any questions the patient may have and review sedation instructions (also available below) with the patient. Have your patient read the consent carefully, initial each paragraph on the line on the left, and obtain the necessary signatures.
3. You will need to handle any outstanding financial issues prior to the apppointment. Once the sedation is started, your patient will be unable to make any legal or important decisions. We strongly recommend prepayment and this appointment is a good time to settle all financial matters.
